🧠 What Is Rabies?

Rabies is a deadly viral disease transmitted through the bite or scratch of an infected animal (commonly dogs, bats, cats, wild animals). Once clinical symptoms appear, rabies is almost always fatal — so immediate treatment is critical.

🧪 Post-Exposure Prophylaxis (PEP): Overview

After any potential rabies exposure:

1) Wound Care

  • Wash bite wound thoroughly with soap and running water for 15 minutes.
  • Use antiseptics (e.g., povidone-iodine) if available.
(Cleaning wounds significantly reduces infection risk.)

📊 Bite Category Classification

Exposure TypeAction Needed
Category I (touching or licking intact skin)No rabies PEP
Category II (nibbling of uncovered skin, minor scratches)Rabies vaccine, no RIG
Category III (single or multiple transdermal bites/scratches)Rabies vaccine + RIG

RIG (Rabies Immune Globulin) gives passive immunity until the vaccine triggers your own antibody production.

đź“… Vaccine Schedules

For people never vaccinated before:

✔ 5-Dose IM Regimen
Vaccines given on: Day 0, 3, 7, 14, 28
This is standard for most rabies PEP protocols.

✔ 4-Dose IM Regimen (Alternative)
Sometimes used in adults without immune compromise:
Day 0, 3, 7, 14 (no Day 28) — depends on guidelines in your region.

For previously vaccinated people:
✔ 2-Dose Protocol
Vaccine on Day 0 and Day 3 (no RIG).

đź’‰ Where to Inject

  • Adults & Older Children: Deltoid muscle (upper arm).
  • Young Children: Anterolateral thigh.
  • Avoid: Gluteal area (ineffective).

🧪 About Rabies Immune Globulin (RIG)

  • RIG should be given once at the start for Category III exposures.
  • It is injected around the wound and intramuscularly at a distant site.
  • RIG should not be given if the person has previous rabies vaccination.

🧾 Common Side Effects

  • Mild symptoms at vaccine site: pain, redness, swelling
  • Rare: fever, headache, fatigue
These are generally mild compared to the risk of rabies.

📌 Important Notes

  • Rabies vaccine does not provide lifelong immunity from one series — boosters are given only if exposure occurs again.
  • Pregnancy is not a contraindication — treat exposures promptly.
